Medicare Advantage plans Missouri residents can choose in 2027

CMS published a Missouri fact sheet on September 28, 2026. It says 1,371,496 Missourians are enrolled in Medicare. Here are its figures for the Medicare Advantage plans Missouri residents can buy, plus drug plans:

Measure (Missouri)20262027
Medicare Advantage plans available157174
Average monthly Medicare Advantage premium$8.19$4.24
Lowest monthly Medicare Advantage premium—$0
People with access to a $0-premium plan—100%
Stand-alone Part D drug plans—9
Lowest stand-alone Part D premium—$5.30
Stand-alone Part D members with a cheaper plan available than in 2026—100%
Stand-alone Part D members who get Extra Help—21.16%

A dash means the fact sheet gives no 2026 figure. Figures are from the CMS Missouri fact sheet as of September 28, 2026, and are shown for illustration only. They are not quotes or offers, and they can change. Confirm current numbers with Medicare or a licensed agent.

Missouri gains 17 plans while the national total slips from 5,553 to about 5,532, according to CMS’s 2027 press release. The Missouri average premium nearly halves, from $8.19 to $4.24, far below the projected national average of $12.00. CMS also says every Missourian in a stand-alone drug plan can find one with a lower premium than in 2026.

Hypothetical example: a Missourian pays the state’s average Medicare Advantage premium both years. In 2026 that is $8.19 x 12 = $98.28. In 2027 it is $4.24 x 12 = $50.88, a saving of $98.28 − $50.88 = $47.40. On Original Medicare, the cheapest stand-alone drug plan costs $5.30 x 12 = $63.60 a year, before Medigap and Part B. Averages hide a wide range, so compare actual plans at your ZIP code.

If your Missouri Medicare Advantage plan is not renewing

A growing market can still drop individual plans. If you received a non-renewal letter, your plan ends December 31, 2026. You can choose among the other Medicare Advantage plans Missouri carriers sell in your county, move to Original Medicare with Medigap and a drug plan, or do nothing. Doing nothing returns you to Original Medicare on January 1 with no drug coverage, which is rarely wise.

Medicare.gov says that when a plan’s contract isn’t renewed, you can switch from December 8 to the last day of February. Choosing by December 31 avoids a gap. Our guide on what to do when a Medicare Advantage plan is not renewing covers each step. If your plan leaves Medicare or stops serving your area, Missouri’s guide says you can buy Medigap Plan A, B, D, G, K or L from any company selling in the state.

The Missouri Medigap anniversary rule

The Department of Commerce and Insurance’s Missouri Medigap Shopping Guide (revised March 2025) describes a yearly switching right. You can switch insurance companies each year during the 30 days before or after your policy’s anniversary date. The guide’s example: if your policy expires June 30, you can switch between June 1 and July 30. Call your insurer to confirm your anniversary date.

For the same lettered plan, the guide says the new insurer cannot deny you coverage or impose a pre-existing condition waiting period. Moving to a more extensive plan will likely mean underwriting. Proof is simple: a renewal notice, invoice or old policy. Cancel your old policy only after the new one arrives. If an insurer says you don’t qualify, call the DCI Insurance Consumer Hotline at 800-726-7390.

The Missouri Medigap anniversary rule applies to people who already have Medigap. It does not move you from Medicare Advantage to Medigap. Once you hold a policy, though, it lets you shop for a better price every year. Our Medicare supplement birthday rule guide compares Missouri’s approach with birthday-rule states.

Leaving Medicare Advantage for Medigap in Missouri

Outside protected windows, Missouri Medigap insurers can ask health questions. The DCI guide lists eight guaranteed issue situations in which a company cannot deny you a policy. Four involve Medicare Advantage:

  • Your plan leaves Medicare or stops serving your area. You can buy Plan A, B, D, G, K or L from any insurer selling in Missouri, if you switch to Original Medicare.
  • You joined Medicare Advantage when first eligible at 65 and switch to Original Medicare within the first year. You can buy any Medigap policy sold in Missouri.
  • You dropped Medigap to try Medicare Advantage and want to return within a year. You can get your old policy back if the same company still sells it, or Plan A, B, D, G, K or L.
  • Your plan broke the rules or misled you. You can buy Plan A, B, D, G, K or L.

Keep the letters that prove your situation. Our Medicare supplement underwriting guide explains what insurers ask outside these windows.

Missouri Medigap under 65

Missouri Medigap under 65 is guaranteed in one key moment. The DCI guide says everyone under 65 who has been approved for Social Security disability has the guaranteed right to buy Medigap when they enroll in Part B. The cost may differ from policies sold to seniors, and the DCI rate materials show under-65 prices separately.

When disabled policyholders turn 65, the guide says they get a second open enrollment period. They can then choose any plan from any insurer and pay the same rates as other people with Medicare. If you are under 65 and comparing Medicare Advantage plans Missouri carriers offer, ask SHIP or DCI how this window applies to you before you choose. Missing it can make Medigap harder to get later.
